Can the applicant's representative collect the delivery instruction slips (DIS)?
Yes, an authorised
representative can collect the delivery instruction slips (DIS) on behalf of
the applicant. The representative must have an Authority letter (refer to
Annexure B, DIS booklet) and the applicant’s self-attested ID proof copy The
DIS booklet for applicants can be collected from Sun Infotech Park, 1st Floor,
Plot No. B-23, Road No 16V, MIDC, Thane Industrial Area, Wagle Estate, Thane,
Maharashtra - 400604.
Note: The DIS requisition form
should be duly filled with all the information. The Annexure B available on the
last page of the existing DIS booklet, and the applicant’s ID proof must be
signed by all the account holders.
